Option 1 (First time mapping): I wish to seed my account with NPCI mapper to enable me to receive Direct Benefit Transfer (DBT) including LPG Subsidy from Govt. of India.

Option 2 (Change mapping): I already have an account with another bank seeded with NPCI Mapper. I request you to change my NPCI mapping to my account with this Bank.

Option 3 (Do not change): I already have an account with another bank seeded with NPCI Mapper. I do not want to change my NPCI mapping from the existing Bank.

Option 4 (No DBT): I do not wish to seed my accounts with NPCI Mapper (I will not be getting DBT).
